start and end from HQ at AMK

Salary:

$60 per case Weekday, $65 per case Weekends

Working Hours:

Min 3-5 days a week, 830-6pm

Key Responsibilities:
. Provide nursing care to patients in their homes
. Conduct patient assessments and care planning
. Monitor patients conditions and document care provided
. Educate patients and caregivers on care management
. Other ad hoc duties as assigned

Requirements:

. Valid SNB and BCLS

. InterRAI Certified

. Relevant nursing experience in community care, home care, or related settings would be advantageous
