Contract Duration: Until end of August (extension to be discussed)
Working Location: Either NTF or NUH. Will confirm working location during interview with candidate.
Work Setup: Home visits around West region. Transport is claimable.
Working Hours: 1 working day per week. Working hours to be confirmed during interview.
- Collaborate with clinical, allied health, operational and administrative stakeholders in planning of community-based occupational therapy services
- Assess and manage patients referred for community occupation therapy independently and implement appropriate treatment plans, applying specialised occupational therapy knowledge and skills
- Coordinate patient care with multidisciplinary teams across hospital and community settings to support care transitions
- Degree in Occupational Therapy recognised by Allied Health Professions Council (AHPC)
- Full Registration with AHPC
