Homecare Staff Nurse (5days work week, Good working environment) #HCL

Key Responsibilities

. Assess new clients and develop personalised care plans.

. Perform nursing procedures (wound care, tube/catheter care, stoma care, medication management, monitoring vital signs).

. Conduct health screenings and make referrals when needed.

. Provide health education to clients and caregivers.

. Coordinate care with doctors, allied health and external partners.

. Build rapport with clients and families.

. Support self-management of health conditions.

. Provide tailored health education.

. Case Management & Administration

. Plan and facilitate Homecare services with the team.

. Maintain accurate client records and reports.

. Manage client workload through scheduling.

. Recognise early warning signs of deterioration.

. Provide appropriate emergency or crisis care.

. Handle enquiries about Homecare services.

. Monitor and manage medical supplies and equipment.

. Support training and development of community partners and volunteers.

. Assist in community outreach and partnership activities.

Job Requirements:

. Min Diploma in Nursing and registration with the Singapore Nursing Board.
