Responsibilities

• Deliver home-based nursing care requiring specialized skills. Plan, execute, and evaluate person-centered nursing care plans tailored to individual client needs. Conduct initial assessments and biannual reviews efficiently and within set timelines. Train caregivers on-site at clients residences. Provide risk assessment and mitigation advice based on anticipated client needs. Manage patient workload through effective scheduling aligned with client requirements. Ensure compliance with infection control measures in line with established policies. Document care activities accurately and promptly after visits, maintaining high professional standards. Participate actively in multidisciplinary team meetings and collaborate in team discussions.

Requirements

• Registered with the Singapore Nursing Board with a valid practicing certificate. Minimum of 4 years ongoing nursing experience in a Medical or Geriatric setting within the last 2 years (experience in Oncology or Palliative Care is an asset). Strong communication and interpersonal skills comfortable engaging with diverse individuals. Demonstrated adaptability and collaboration as a team player, with responsiveness in various communication channels. Independent, reliable, and capable of making sound clinical judgments in home-care settings. Keen interest in providing End of Life care for homebound seniors. Availability to support After Office Hour care for End of Life patients on a rotational basis (Tele-consult).
