HOME CARE STAFF NURSE (PALLIATIVE CARE ) #HMS

Home Care Nurse will be responsible for the provision and maintenance of quality and safe clinical services to home care clients.

Monday - Friday OFFICE HR
On call duties (only tele-support, answer client texts)

Responsibilities:

Delivery of safe care and sound clinical decision making

Assessing and documenting observations of clients condition at each visit

Initiating/ reviewing nursing care plan based on assessment made

Maintaining accurate nursing documentation

Performing necessary nursing procedures such as wound dressing, urinary catheter insertion, nasogastric tube insertion, medication packing and etc.

Conducting regular evaluation of care provided for home care services

Provide caregiver training and education to client including families on home health care procedures and strategies

Reporting of any problem incidents encountered in a client's condition and/or in handling clients and/or caregiver to Nurse Lead
