This role involves conducting comprehensive care assessments and engage residents in health goal-setting. Implement lifestyle interventions to reduce health risks, support chronic disease self-management, and improve quality of life through sustainable behaviour change. Collaborate with the Community Health Team to ensure holistic care planning and effective service delivery.
Job Responsibilities
- Assess residents health status and risk factors through subjective and objective evaluation
- Facilitate collaborative goal-setting with residents and stakeholders using S.M.A.R.T. framework
- Design tailored health coaching plans to guide residents towards wellness goals
- Support residents in addressing lifestyle and behavioural change challenges
- Track and document resident progress and health outcomes
Job Requirements
- Diploma/Advanced Diploma in healthcare-related field
- Ability to work independently and as part of a team
- Strong communication and social engagement skills, particularly with older adults
- Excellent organisational, coordination, and interpersonal abilities
- Proficiency in computer skills and Microsoft Office applications
