Job Description

Overall Job Responsibility

The Manager Supports The FCPRC's Efforts To Develop The Ecosystem Of Support For Future Care Planning (FCP). These Efforts Include, But Are Not Limited To, The Following

  • Resource development for FCP;
  • Capability building of sector professionals to deliver FCP to their clientele;
  • Support caregivers in FCP through triaging and routing of cases to relevant sector professionals;
  • Community engagement, public education and targeted outreach on FCP amongst caregivers and key stakeholders;
  • Building collaborations with stakeholders/enablers within SG Enable and external agencies from relevant sectors to facilitate the FCP process.

The Manager also develops and implements other initiatives to fulfill the role of the FCPRC and achieve its goals.

Duties and Responsibilities

  • Resource Development
  • Develop FCP outreach and public communications materials (brochures, toolkits, human interest stories for media etc.).
  • Collaborate with stakeholders to develop and deliver engaging content.
  • Manage social media platforms to ensure they deliver consistent and impactful messaging to caregivers and sector professionals. This includes maintenance of the Special Needs Trust Company website.
  • Building Sector Capability
  • Secretariat work to support Resource Panel
  • Support Caregivers
  • To attend to walk-in/phone-call/email enquiries, MP appeals on Future Care Planning
  • Triage and direct referrals on Future Care Planning to sector parters or SGE's case management team
  • Community Engagement and Public Education
  • Develop and implement community engagement strategy to raise awareness on future care planning (FCP): Plan and coordinate community engagement and public education events/activities. This includes organising workshops and conducting presentations on FCP.
  • Sector Collaboration
  • Support FCPRC's efforts to develop and maintain partnerships with key stakeholders from FCP-related fields (legal, housing, financial planning, healthcare/ageing), ranging from government and private sector agencies to social services and community-based agencies.
  • FCPRC Operations
  • Responsible for the day-to-day running of the FCPRC, including oversight of logistics, space utilisation, staff coordination, public inquiries, service delivery etc.
  • To provide administrative and logistical support for training and case management sessions conducted in FCPRC
  • Support Group-level projects or initiatives related to FCP, as assigned by team leaders.

Qualifications, Knowledge And Experience

  • A degree holder with at least 5 years of relevant work experience, preferably in the social service/disability sector, case management, operations management, and/or public education.
  • Strong relationship-building and interpersonal skills. Proficiency in community outreach/engagement, with the ability to work effectively across diverse stakeholders.
  • Excellent communication skills both written and verbal to engage different stakeholders, drive alignment and get buy-in.
  • Creative problem-solving skills, and experience in programme development and management.
  • Detail-oriented and proficient in tools like Outlook, MS Word, Excel, PowerPoint, and project management software.
  • Familiarity with digital outreach platforms and social media management for caregiver engagement.
  • Customer service, public speaking skills and knowledge of social service case management processes, is an advantage.
  • Experience in developing public education resources and information guides/playbooks/toolkits, is an advantage.
