JOB SCOPE/KEY RESPONSIBILITIES
Summary: Support programme in promoting clients health, safety and well-being through the provision of nursing care, health monitoring and daily living support. Support the team to achieve improvement in clients participation, independence and overall well-being through a collaborative and person-centred approach.
- Assist in providing nursing care, medication administration and health monitoring for clients in accordance with nursing standards and organisational policies.
- Assist programme staff in supporting clients with activities of daily living, including feeding, grooming, toileting, personal hygiene, transfers and mobility, and maintaining good standards of care and hygiene.
- Assist in monitoring clients health conditions and progress, maintaining relevant documentation, and reporting concerns or changes in condition to the Manager.
- Assist the team in implementing individual care plans and supporting clients health-related, rehabilitation and programme participation needs.
- Assist in coordinating medical follow-ups, healthcare appointments and communication with caregivers and healthcare professionals regarding clients care needs.
- Assist in responding to medical emergencies and ensure emergency equipment, medical supplies and consumables are properly maintained and available when required.
- Assist in implementing infection prevention and control measures and ensure that centre policies relating to clients health, safety and well-being are adhered to.
- Assist the team by providing health-related information, observations and feedback to support multidisciplinary care planning and service delivery.
- Carry out proper and regular housekeeping of medical equipment, treatment areas and related facilities.
- Report to Manager on any accident, injury, illness or health-related incident involving clients during programme activities.
- Any other duties as and when necessary.
B JOBHOLDER REQUIREMENT:
- Minimum ITE Certification, preferably with at least 1 years of experience in working with persons with disabilities.
- Registered with the Singapore Nursing Board (SNB) as an Enrolled Nurse
- A self-motivated team player, adaptable and able to multi-task.
- Good communication and people management skills as well as technical ability.
- Meticulous and strong observational skills.
