We are looking for a passionate and skilled nurse to join our Primary Care Network. You will play a key role in helping patients manage diabetes and other chronic conditions, while growing your own professional skills in a supportive and collaborative environment.
About the Role:
Lead chronic disease management through nurse counselling, diabetic screenings (DFS/DRP), and patient education. Mentor junior nurses, develop clinical protocols, and drive quality improvement in a collaborative primary care setting.
What You'll Do:
- Provide structured nurse counselling using motivational interviewing.
- Educate patients and caregivers on chronic disease management, health promotion, and prevention.
- Monitor patient progress with skill-specific tests (blood glucose, blood pressure, etc.) and track lifestyle improvements.
- Conduct Diabetic Foot Screening, Diabetic Retinal Photography, and spirometry services (as needed).
- Identify high-risk cases and coordinate with doctors for timely interventions.
- Develop and implement structured care plans and participate in case discussions.
- Mentor and support newly hired nurses and clinical care coordinators.
- Contribute to training programs, competency assessments, and review nursing guidelines.
- Collaborate across clinics, public institutions, and community partners to enhance patient care.

Who We're Looking For:
- Registered Nurse with experience in chronic disease or community care (Diabetes experience preferred).
- Strong communication and mentoring skills.
- Passionate about improving patient outcomes and working collaboratively.
