We are looking for a dynamic individual to join our Senior Connect team to serve the seniors in the community, pursuing productive ageing lifestyle.
Job Description:
1. Programme Coordinator for Senior Activities
- Coordinate and oversee the weekly Senior SWEE! programme, including planning, execution, and evaluation of activities.
- Develop and pilot new programmes and activities aligned with BCS's Model of Care, responding to emerging needs of seniors.
- Lead outreach efforts to expand the geographical coverage, enabling BCS to reach and serve more seniors within the community.
2. Community Outreach
- Coordinate Community Outreach programmes (e.g. Neighbourhood Visitations) to identify, engage, and connect with individuals and families in need.
- Follow up and coordinate outreach initiatives arising from Neighbourhood Visitations, ensuring appropriate support, referrals, and continuity of care.
3. Community Pantry
- Oversee the operations of The 3rd Stop Pantry (Food rescue and re-distribution to the needy), ensuring smooth day-to-day functioning.
- Grow and sustain the pantry through community partnerships, volunteer coordination, and resource stewardship to better serve beneficiaries.
