Community Nurse (Homecare)

Join our dedicated team as a Community Nurse, where you will play a vital role in delivering quality care and support to the elderly in the community. Your expertise will be instrumental in patient education, infection control, and guiding care staff.

Responsibilities

• Plan, coordinate, and integrate care for the elderly within the community. Conduct patient assessments and perform necessary clinical procedures. Collaborate with the Nurse Manager to deliver practical training and competency evaluations for care and nursing staff. Develop and implement individualized care plans, ensuring comprehensive monitoring and evaluation of patients. Provide specialized education and training for patients and caregivers, including individual and group learning programs. Evaluate the effectiveness of patient education and communicate treatment plans to patients and their families. Maintain standard precautions and oversee adherence to infection control protocols, educating patients and families on these practices. Offer clinical guidance to Junior Enrolled Nurses, Nursing Aides, and Care Associates regarding patient care delivery. Pursue continuous professional development through ongoing training and research linked to registration requirements. Perform additional duties as assigned by the supervisor.

Requirements

• Diploma in Nursing is required. Advance Diploma in Geriatrics, Medical-Surgical, Chronic Disease Management, or Community Health is preferred. Must hold a valid practicing certificate from the Singapore Nursing Board. Current BCLS & AED certification/recertification is necessary. At least 2 years of nursing experience in a community care setting in Singapore. Knowledge and understanding of community care practices.
