My client is seeking an experienced healthcare professional to lead a team focused on post-discharge care coordination and community integration. This role involves strategic planning, team leadership, and cross-sector collaboration to improve patient outcomes and reduce unnecessary hospital readmissions.
Key Responsibilities:
- Lead and mentor a team of care coordinators supporting patients and caregivers after discharge
- Oversee follow-up care through phone outreach and home visits, ensuring patients can safely transition back into the community
- Coordinate with healthcare partners to streamline services and strengthen support systems
- Develop and implement frameworks that enhance community care delivery
- Supervise clinical performance and guide professional development for team members
- Monitor data quality and contribute to service improvement initiatives

Requirements:
- Professional background in Nursing, Social Work, Allied Health, or related clinical fields
- Minimum 8 years of experience in care coordination or community health
