Job Description

Reporting to the Cluster Manager, the Community Care Executive works closely with the Cluster Manager to manage the daily operations of the AAC, so that we can deliver the mission of providing quality care, bring joy and comfort and honour the dignity of seniors in a caring environment.


Key Responsibilities:

1. Day-to-Day Centre Operations

  • Ensure smooth and optimal centre operations, such as maintaining fixed assets, IT devices and equipment, upkeeping cleanliness and hygiene in accordance with national standards and handle related facility management issues.
  • Work closely with Cluster Supervisor and the team to ensure adherence to operational directives, standards and indicators issued by the various government agencies (MOH, AIC, MSF) and Senior Management, as well as preparing the centre for audits.
  • Assist the Cluster Supervisor in monitoring and guiding staff performance to ensure smooth operations and compliance with set standards.

2. Programme Planning, Implementation and Evaluation

  • Outreach and engagement: i) conduct regular outreach to seniors in identified HDB blocks/ private residences and carry out regular visits to vulnerable and socially isolated seniors to monitor their health and well-being, ii) promote and advertise the activities to seniors in the designated blocks, iii) conduct Community Screener Tool (CST) for vulnerable seniors and facilitate necessary referrals for service linkups.
  • Develop and implement an annual calendar plan of health and social activities for seniors, taking into account the 2S KPIs, as well as advocating a range of activities such as intergenerational, outdoor and interest-based mutual self help groups.
  • Assist in collecting data and collating ground feedback for programme evaluation and review purposes.

3. Volunteer Management and Community Partnerships

  • Drive and contribute to ASH's volunteer management framework, by actively recruiting, orientating, training, retaining, deploying, engaging and appreciating volunteers in alignment with the assigned roles, mission and objectives of the AAC.
  • Represent ASH in community partnership networking sessions, such as Communities of Care, multi-agency case discussions and meetings.

4. Basic Case Management and Service Referrals

  • Befriending and Buddying: familiar with the reporting requirements and collaborate with the team to manage the intake and referral process for seniors needing the services.
  • Care Information and Referrals: knowledgeable with a range of schemes and grants for seniors and their caregivers, so as to answer ground queries, as well as provide effective and efficient service referrals to the relevant agencies or departments for follow-ups.

5. Data Reporting and Administration

  • Tracking of AAC KPIs such as administering CST, programme participation attendance, vital signs records.
  • Ensure data confidentiality and strict adherence to Personal Data Protection Act (PDPA) guidelines, as well as proper and systematic storage, use, handling and disclosure of personal data.

6. Other Duties

  • Participate in organisation-wide projects, taskforces and initiatives that advance and support the interests of AACs
  • Any other duties as assigned by Reporting Officer (Cluster Manager)

Job Requirements:

  • Minimum Diploma in any discipline from an accredited Polytechnic. Preferably in the field of Gerontology, Social / Healthcare Services, Community Development, Psychology, Social Work, Counselling.
  • At least one (1) year of experience in front-facing, direct operational work in eldercare or social services. Candidates with prior volunteering experiences with the elderly are preferred.
  • Proficient in IT and Microsoft Office applications.
  • Good interpersonal skills: resourceful, proactive, responsible and client-centric. Passionate in serving seniors and volunteers, preferably able to speak Chinese dialects.
  • Good team player yet can work independently, ability to multi-task and meet deadlines in a fast-paced environment, have good planning and organisational skills.
