Case Manager

The Case Manager provides professional counselling, case management and intake services to individuals and families. The role involves assessing clients needs, developing and implementing intervention plans, providing follow-up, and working with community partners and multidisciplinary teams to support positive outcomes.

The role also supports programme development, outreach, training, mentoring and consultancy, while maintaining proper case documentation and service records.

KeyResponsibilities

Counselling& Case Management

  • Conduct intake and consultation, counselling and casework in accordance with organisational guidelines and professional standards.
  • Conduct assessments and provide appropriate counselling and case interventions based on clients needs.
  • Manage an assigned caseload independently, including follow-up and case closure.
  • Support clients in crisis situations and escalate cases to supervisors when required.
  • Develop and implement intervention plans based on professional judgement.
  • Collaborate with relevant agencies, community partners and multidisciplinary teams in case management.
  • Participate in clinical supervision and case discussions.

ProgrammeDevelopment & Outreach

  • Develop and implement preventive, outreach and support programmes that complement counselling services.
  • Collaborate with community stakeholders and volunteers in programme planning and implementation.
  • Monitor programme outcomes and identify opportunities for service improvement.
  • Support mentoring, training and consultancy initiatives.

Administration& Professional Practice

  • Maintain accurate case records, client databases and programme documentation.
  • Prepare and submit required reports and statistics.
  • Provide guidance or supervision to interns, new counsellors and support staff where assigned.
  • Undertake other duties and projects as assigned.

Requirements

  • Degree in Counselling or a relevant discipline.
  • Minimum 2 years of relevant clinical counselling or case management experience, preferably in motivational, marital or couple counselling.
  • Graduate Diploma in Couple and Family Therapy is an advantage.
  • Good communication, interpersonal and stakeholder management skills.
  • Self-motivated, collaborative and committed to supporting individuals and families.
  • Passion for making a positive difference in the lives of individuals and families.
  • Salary commensurate with experience.
