Serve as a clinical resource for the healthcare team and document care management activities. Formulate and revise guidelines, standards, and procedures related to disease management. Conduct audits of documentation and identify areas for improvement. Participate in research and quality improvement initiatives, applying findings to enhance practice. Oversee project management tasks, promote professional development, and provide feedback for care managers. Supervise junior care managers and conduct training to develop future nursing leaders. Monitor productivity and track the performance of care management nurses. Coordinate recruitment and education initiatives for patient workshops and support groups. Attend forums and participate in committees to stay updated on best practices in disease management.

Requirements

• A relevant degree in healthcare or a related field. Proven experience in care management and clinical assessment. Strong analytical skills, with the ability to conduct data analysis for quality improvement. Excellent organisational and leadership abilities.
