RESPONSIBILITIES
The Assistant Nurse Manager (HPC & HPC+) supports the Nurse Manager in providing clinical leadership, governance and operational oversight for Home Personal Care (HPC) and Enhanced Home Personal Care (HPC+) services. The role combines hands-on community nursing practice with team supervision and service coordination, ensuring safe, ethical and person-centred care for clients with complex functional, medical and end-of-life needs in the community.
JOB RESPONSIBILITIES
- Clinical Care and Service Delivery
- Provide direct nursing care and clinical oversight for clients receiving HPC and HPC+ services, including those with complex care and end-of-life needs
- Support care planning, clinical decision-making and escalation in accordance with organisational protocols and scope of practice
- Monitor care delivery and client outcomes to ensure safety, quality and continuity of care in the community
- Ensure accurate, timely and compliant clinical documentation and records
- Team Supervision and Support
- Support the Nurse Manager in supervising nursing staff, including guidance on clinical practice, workload management and adherence to standards
- Provide on-the-ground clinical support, coaching and mentoring to nurses and care staff
- Assist in competency assessments, onboarding and training of nursing staff
- Escalate complex clinical, staffing or risk issues to the Nurse Manager in a timely manner
- Care Coordination and Collaboration
- Work collaboratively with multidisciplinary teams, community partners and service providers to support integrated care delivery
- Participate in case discussions and care conferences to support coordinated and safe community care
- Support care transitions, step-down care and service continuity to reduce avoidable hospital admissions
- Quality, Safety & Professional Practice
- Assist in quality improvement, audit and service evaluation initiatives within HPC and HPC+ services
- Support clinical risk management, incident reporting and follow-up actions
- Uphold professional, ethical and regulatory standards in accordance with the Singapore Nursing Board requirements
QUALIFICATIONS- Bachelor's Degree in Nursing or equivalent recognised qualification
- Registered Nurse with the Singapore Nursing Board (SNB)
- Post-basic qualification or training in community nursing, gerontology, palliative care or leadership is preferred
OTHER INFORMATIONRelevant Experience
- Minimum 4-6 years of nursing experience, with experience in community, eldercare or home care settings preferred
- Prior experience in a senior staff nurse, nurse clinician or supervisory role is an advantage
- Demonstrated interest in leadership, service development and community nursing practice
Competencies
- Strong foundation in community nursing, eldercare and home-based care
- Ability to guide, support and mentor nursing staff in daily practice
- Competence in identifying risks, escalating concerns and supporting safe care delivery
- Effective communication and documentation skills, possesses teamwork with multidisciplinary and community partners
- Strong understanding of professional standards, accountability and scope of practice
