Job Description

About the Role

We are looking for an experienced nurse to take on a leadership role in delivering high-quality, home-based care for clients with complex medical and functional needs.

This role combines hands-on clinical care with team supervision and care coordination, supporting clients in the community including those with chronic conditions and end-of-life care needs.

You will play a key role in ensuring safe, person-centred care while mentoring nursing staff and strengthening service delivery.

Key Responsibilities

Clinical Care & Service Delivery

  • Provide direct nursing care for clients in home-based settings

  • Support care planning and clinical decision-making for complex cases

  • Monitor care outcomes to ensure safety, quality and continuity

  • Maintain accurate and compliant clinical documentation

Team Leadership & Supervision

  • Support supervision of nursing staff and daily operations

  • Provide on-the-ground guidance, coaching and mentorship

  • Assist in onboarding, training and competency development

  • Escalate clinical risks and operational issues where required

Care Coordination & Partnerships

  • Work with multidisciplinary teams and community partners

  • Participate in case discussions and care planning

  • Support care transitions and continuity across services

Quality, Safety & Governance

  • Support audits, quality improvement and service evaluation

  • Manage incident reporting and follow-up actions

  • Ensure compliance with clinical and regulatory standards

Requirements

  • Bachelor's Degree in Nursing or equivalent

  • Registered Nurse with Singapore Nursing Board (SNB)

  • 4-6 years of nursing experience (community / eldercare preferred)

  • Prior supervisory or senior staff nurse experience is an advantage

  • Strong clinical foundation in community or home care

  • Ability to mentor and guide nursing staff

  • Good risk assessment and escalation capability

  • Strong communication and documentation skills
