Job Description:

• Lead planning and coordination of community care services to ensure efficient operations and optimal resource utilization
• Collaborate with multidisciplinary teams and stakeholders to align care delivery and service planning
• Analyze data and trends to forecast demand and improve operational efficiency
• Monitor performance metrics and drive continuous improvement initiatives
• Develop and implement programs to enhance community health outcomes
• Ensure compliance with healthcare standards, policies, and regulatory requirements
  

Requirement:

• Minimum Business Administration, Public Administration or Healthcare Administration or equivalent
• Minimum 5-7 years of relevant working experience
