Responsibilities
- Facilitate patient selection, counselling, enrolment and referrals to primary care partners. Provide training and develop workflows to assist hospital departments in right-siting efforts. Support continuous patient care within the community. Monitor and manage the transition of patients from polyclinics to general practitioner partners. Input and maintain accurate data in the tracking database. Engage patients in health promotion activities and national initiatives as needed. Execute additional tasks as assigned by supervisors and the Head of Department (HOD).
Requirements
- Possess a Diploma in a health or social-related discipline. Demonstrate the ability to multi-task and work autonomously as well as collaboratively. Exhibit strong organisational, coordination, interpersonal, and communication skills. Respond effectively to inquiries or complaints from doctors, staff, and patients. Proficient in Microsoft Office applications.
