As a Assistant Care Coordinator (Healthcare), you will be responsible for the following duties:
- Conduct pre-admission screening and assess patients care needs.
- Support nurses and healthcare professionals in providing holistic home-based care.
- Monitor patients conditions and identify concerns requiring escalation.
- Develop and update individualised care plans based on patient needs.
- Communicate with patients and caregivers regarding care plans and enquiries.
- Monitor vital signs remotely using telehealth systems and digital platforms.
- Provide basic health education and lifestyle coaching on diet, nutrition and exercise.
- Maintain accurate documentation of patient care, progress and treatment plans.
- Coordinate with healthcare providers, community resources and vendors to ensure continuity of care.
- Participate in quality improvement, risk management projects and other assigned duties.
Requirements:
- Minimum Diploma in Health Promotion or healthcare-related disciplines.
