Work Location: West side polyclinics

Job Responsibilities:

Deliver safe and effective nursing care for patients with acute and chronic medical conditions.
Perform patient assessment, triage, treatment and follow-up care.
Support nurse-led services in chronic disease management.
Provide health education, preventive care, vaccinations and lifestyle counselling.
Administer medications and treatments in accordance with clinical protocols.
Collaborate with physicians and allied health professionals to ensure coordinated patient care.
Maintain accurate clinical documentation and adhere to nursing and infection control standards.

Requirements:

At least 2 - 3 years Staff Nurse experience, in community/primary healthcare settings will be a strong advantage
Registered Nurse with Singapore Nursing Board
BCLS & AED Certified
Degree or Diploma in Nursing
